The Application Form for new projects and programs, and the Pilot Project Application Form are only available by contacting DMC. The Renewal Application Form is made available here for projects receiving ongoing funding.

Application Form
Applications for Operation Manna funding for new projects and programs must be submitted to Diaconal Ministries Canada by August 15.

Renewal Application Form
Current Operation Manna partners must submit renewal applications to Diaconal Ministries Canada by November 15.

Pilot Project Application Form
Applications for pilot projects or programs may be submitted to Diaconal Ministries Canada at any time during the year.

Evaluation Form
Operation Manna recognizes and promotes the need for ministries to regularly assess their organizational capacity in order to celebrate what was done well and to identify areas that need growth or attention. Operation Manna has developed a user-friendly monitoring tool that relates ministry growth to the growth and development of a plant (from seed to fruit bearing).
